This is a mandatory form required to claim the government's Early Years Funding entitlement for your child (30 hours funding).

Important:

• If you are claiming 30 hours, you must submit this form every time your eligibility code has been reconfirmed by the government, even if the change is only to the expiry date.
• Failure to submit the required information or reconfirmed code on time may result in your funding being forfeited for the term.
