Please complete this form ONLY if you have been approved for a Tax-Free Childcare (TFC) account and wish to use it to pay your child's fees.

Important:

• You must provide your child's TFC Payment Reference (usually four letters and five numbers, e.g., AJON12345TFC) and your TFC Account Number (13 digits, starting with 1100).
• We require this information to link the incoming government payments to your child's nursery account. Without it, we cannot identify your payment.
• Once submitted, please ensure you set up the payments to the nursery from your TFC account via the Government portal. Payments may take up to 24 hours to clear.

This form is only required once per child unless your TFC details change.
